Instructions for DISC On-Line Scholarship Application

The application form has been tested with Internet Explorer and Firefox.  It does  not currently work with the AOL browser.  Other browsers have not been  tested.


DISC implemented an On-Line Scholarship Application to help ease the processing of applications.  There is still some paperwork that has to change hands but this should simplify the process.

As it says above, this is the BETA version on the application.  DISC has tested it among ourselves but as with any new implementation, we probably didn’t identify all of the issues.  We appreciate your patience.  If you have any problems contact discwebmaster@idodi.org.  We are still working on improving the error messages. 

You will be able to work on your on-line application up to the scholarship deadline of February 1, 2010.  HOWEVER, you need to remember to mail all required application materials (signature page, CD or DVDs, etc.) in time to arrive on or before February 1, 2010.

Step 1: Information required for Completing your Scholarship Application. (Be sure you know all of this before starting the Application.) 

TAB 1 -General Information

Applicant ID
This is an ID that you create to identify and retrieve your Application.  It must be at least 10 characters long.  We recommend that you NOT use your email address for your Applicant ID.

Tab 2 - Challenges Completed

Number of Years: 
How many years have you been involved with one of the creative problem solving programs sponsored by Destination Imagination, Inc.
Challenge listing 
Using the boxes fill in the year and the challenges you competed in.  Use the text box at the bottom if you competed in more than 10 challenges.

Creativity Awards
Use the text box to list the year and any special awards you or your team received.   List the year, award and why you or your team received the award.
Other Involvement 
Using the boxes, fill in the year and any additional involvement in DI beyond competing in a challenge.  This could include leading training, appraising, serving as a team manager, etc.  If you need more space than provided, use the text box at the bottom of the tab

Tab 4 - Team Application Information

If you are submitting a presentation or internet application as part of a team, use the boxes below to list your other team members.  Each team member must submit a separate application.  Give us the First Name, Last Name and Email address of each of your team members.

Tab 5 - Founder's Award

The Founder’s Award is awarded each year in recognition and appreciation of the founders of VOMBO, the first scholarship booster organization for DI.  The winner is chosen from applicants from any of the three tracks.  The award is given to the applicant who best exemplifies the spirit of VOMBO through volunteering in support of Destination Imagination and its mission.  If you wish to apply for the Founder's Award, tell us in 250 words or less why we should consider you for the Award.

Step 2: On-Line Scholarship Application

DISC has implemented an On-Line Scholarship Application to help ease the processing of applications.  There is still some paperwork that has to change hands but this should simplify the process.   As it says above, this is the BETA version on the application.  DISC initated the system last year but with the relocation to the new site there is always a chance that something "broke" that we missed.  We appreciate your patience.  If you have any problems contact discwebmaster@idodi.org.  We are still working on improving the error messages. 
 
You will be able to work on your on-line application up to the scholarship deadline of February 1, 2010.  However, you need to remember to mail all required application materials (signature page, CD or DVDs, etc.) in time to arrive on or before February 1, 2010.
 
When you open up the form you will see the application and various tabs.  There are 6 control buttons at the bottom of the page:  “Create”, “Retrieve”, “Update”, “Delete”, “Print Application”, “Print Signature Page”
 
CREATE
You use this control to create your Application.  At a minimum you must have input an Applicant ID (see instructions on next page) and your email address.   You should receive an email that indicates that your Application has been created.  If you do not receive an email notifying you that your application has been created you should contact the scholarship committee immediately at scholarshipchairs@idodi.orgIf you click the CREATE control a second time you will see  “Error:  Duplicant entry” and your Applicant ID in single quotes.  This message means that you have tried to create another application with an Application ID that is already in use.  You also cannot create 2 separate applications with the same email account. 
RETRIEVE
This control allows you to retrieve an application that you have already started.  Type in your Applicant ID in the Applicant ID field and click the RETRIEVE button.  If “ERROR” shows up at the bottom of the form then the application was not found.  We have not currently coded the section for retrieval of your applicant id via email.  Until then, if you need help with your applicant id, contact scholarshipchairs@idodi.org and they can help you.  You must email them from the email address you used to create the Application or they will not send you the Applicant ID. 
UPDATE
Use this control to update and save your application as you work on it.  The update control is not available until after you have created your application.  You will get a confirming email listing the time each time you click the UPDATE button.  If you do not receive this email please contact the scholarship committee immediately at scholarshipchairs@idodi.org
 
Remember to click the UPDATE control one last time before you close  the Application Window or you may lose information.
 
PRINT APPLICATION
This control button  is for applicants who would like to see a hardcopy of the information that they have input into the form.  It is strictly a non-formatted listing of all the data you have input.  This is not for submission to the committee.
PRINT SIGNATURE PAGE
This control button brings up a PDF of a form that must be signed and submitted to the committee PRIOR to the Scholarship close Deadline.  You can print this and mail it at any time.  You are welcome and encouraged to scan the signed document and send it via email to scholarshpchairs@idodi.org but we still must have an original signature
